Service agreements
Submitting a form does not create a service contract. Project-specific scope, pricing, warranties, exclusions, payment terms, and other conditions are provided separately in writing before work begins.
Third-party services
Some work may be performed or supported by appropriately qualified independent service providers. The role of MBHS and any third party will be explained as part of the project arrangements.
